In this role, you will: Perform comprehensive medicines reconciliation on admission Deliver medicines optimisation & clinical services during inpatient stays Attend ward rounds/MDTs to support patient care Provide pharmacy authorisation of discharge summaries Work across multiple sites (travel required) Develop innovative practices to enhance patient experience and care Requirements & Qualifications: To be successful in this role, you will need: A current and valid GPhC registration Experience in general medical specialties Strong communication and teamwork skills Ability to work independently and within a multidisciplinary team A commitment to high-quality patient care Working Hours & Contract Duration: This role offers a flexible working position requiring availability from Monday to Friday .
