Job Description

Cityworx are recruiting for an experienced Care Coordinator to join an established domiciliary care business based in the Southampton area. As a Care Coordinator your role is to assist the Registered Manager in organising the day-to-day running of the service using the policies, training, supervision, and guidance provided, supporting the Registered Manager in achieving the aims and objectives and ensuring quality is paramount in the service. Requirements: · NVQ Level 3 in Care (or willingness to work towards) · 1 years experience as a domiciliary care coordinator · Willing to participate in the on-call rota · Full driving licence with use of own vehicle Job Description: To cover all calls on the electronic rota system and ensure weekly rotas are completed in a timely manner. To follow up the initial contact between the care worker and service user to evaluate the Care Plan and ongoing needs. To develop and maintain relationships with your service users and care staff team. Undertaking regular telephone assessments which monitoring the service user’s satisfaction and care worker performance. Effective communication with a wide range of individuals you will liaise with appropriate professionals, clients and the organisation’s management as required to improve care standards · To maximise the capability of the electronic monitoring system with alerts assigned and managed in accordance with contract specific parameters. To maintain accurate written and computerised record keeping To participate in the on-call rota
