About the Role:
As a Care Coordinator, youll play a vital role in ensuring that our clients receive the highest standards of personalised care. Youll manage client needs, support our dedicated carers, and help maintain a nurturing and professional environment. Key Responsibilities:
- Coordinate and manage day-to-day care for our clients, ensuring individual needs are met.
- Build trusting relationships with clients and respond swiftly to their needs.
- Oversee carer placements and contracts, scheduling client rotas for smooth operations.
- Handle complaints with care, working closely with CQC inspectors as needed.
- Regularly update and review client care plans, conduct spot checks, and manage incident reports.
- Maintain effective communication with both clients and carers, supporting continuous improvement.
- Participate in the on-call rota and assist with audits to enhance service quality.
Who Were Looking For:
- Experience & Qualifications: At least one year in a Care Coordinator role within a community setting, with an NVQ Level 3 qualification or equivalent.
- Skills & Personality: Strong communication, organisational, and problem-solving skills, with a genuine passion for providing high-quality care.
- Additional Desirables: Experience with any care planning system and working with professional bodies or inspectors is a plus!
- Driver : Access to a car for client visits is a bonus but not mandatory
