Location: Reading Area
Salary: £25PH to £27PH, depending on experience
Contract: Full-time or Part-time, Immediate Start Available
Remote Working: Partially Remote

Are you a dedicated Band 6 Community Psychiatric Nurse (CPN) or Social Worker looking to make a meaningful difference? A great client of ours is seeking a professional for an Adult Community Mental Health Team, supporting individuals aged 18 to 65 in the Reading area. They offer a flexible work environment with a balance of on-site and remote working.
Role Overview
You will be responsible for managing a caseload of up to 30 service users. You’ll play a vital role in care coordination, delivering high-quality support and interventions tailored to each individual’s needs.
Key Responsibilities
Manage a caseload of up to 30 people aged 18–65 with diverse mental health needs
Provide care coordination and case management within the community
Deliver person-centered assessments, interventions, and care plans
Work collaboratively with other healthcare and social care professionals
Attend team meetings and case discussions
Engage with clients and their families to promote recovery and wellbeing
Requirements
Valid NMC registration for CPN or SWE registration for Social Workers, with no restrictions
Experience in community mental health, ideally in an adult setting
Ability to manage a caseload efficiently, maintaining a high standard of care
A car and valid driver’s license to travel within the community
Strong interpersonal and communication skills
