Intake Case Manager

Location: Hybrid (Main Office: Denver, CO)


Industry: Nonprofit / Human Services


Pay: $22.07 per hour (Starting at $22.07, with a potential increase to $24.52 after 6-month competency review)

About the Role:

We are seeking motivated Intake Case Managers to join our client's team. This role is ideal for individuals passionate about community service and helping others navigate complex systems. You will play a crucial role in managing member cases, ensuring timely assessments, and providing ongoing support to members and their care teams.


Responsibilities:

  • Respond promptly to member, family, or provider inquiries, including questions, concerns, and requests for status updates.
  • Conduct virtual eligibility assessments and quarterly monitoring visits with members.
  • Complete required paperwork, submit certifications, and manage case-related documentation.
  • Coordinate with care teams to find service providers and necessary resources for members.
  • Monitor incident reports and follow through on necessary actions or investigations.
  • Effectively manage a caseload while ensuring all cases comply with organizational and regulatory standards.
  • Assist in case closures or terminations, ensuring members understand the complaint and appeal process.


Qualifications:

  • Bachelor’s degree in social work, psychology, or a related human services field, or 5 years of relevant experience in LTSS, particularly with developmental disabilities.
  • Combination of education and experience will be considered.
  • Preferred: Case management experience and bilingual in Spanish.
  • Experience working with IDD populations, hospitals, or nursing facilities is a plus.


Additional Details:

  • Hours: Monday to Friday, 8 AM - 5 PM during training. Potential to shift to a 4-day, 10-hour workweek once full-time.
  • Tentative Start Date: 2 weeks from offer
  • Training: Comprehensive training provided, including classroom-style sessions, 1-on-1 mentorship, and ongoing support.
  • Software: CaseLogic, Astro, and Care & Case Management Systems
  • Dress Code: Business casual, with flexibility depending on daily tasks.
