We're seeking a skilled Licensed Practical Nurse (LPN) for a 13•week contract role in our home health team. The position requires a 40•hour week, day shift schedule.

Responsibilities:
• Deliver home•based patient care within assigned areas.
• Document patient care services.
• Collaborate with the healthcare team to manage patient care plans.

Requirements:
• Active MA LPN License.
• BLS Certification.
• 2 years of home health nursing experience.
• Ability to float as needed.

• 1:1 patient ratio.
