Job DescriptionJob DescriptionHealth and Well-Being Case Manager- Tierra Solida/Solid Ground General Description: Our focus at CHHOP (Caring for the Hungry and Homeless of Peekskill) is housing and food equity. We operate a shelter for unhoused adults of all genders, including veterans, a collaborative housing and support services program for domestic violence survivors and their families, a permanent supportive housing program for chronically homeless individuals with a disability, and Fred’s Pantry a large “farm to pantry: food pantry. Tierra Solida/Solid Ground: This program will boost housing stability, and the health and well-being of low wage workers and immigrants. Job Summary: The Tierra Solida/Solid Ground Health and Well-Being Case Manager will work with Tierra Solida/Solid Ground participants and will also be available to Fred’s Pantry clients regardless of Tierra Solida/Solid ground participation. - Conduct client intake/assessment
- Obtain authorizations, client releases and required documents.
- Timely enter case notes
- Develop a Social Risk Assessment and determine appropriate resources for a community services and connection plan
- Collaboratively assess client needs and develop short- and long-term goal plan.
- Maintain all program binders and documents as “audit ready.”
- maintain and follow up with client contact as required
- Develop relationships with community-based organizations as potential collaborative partners
- Other duties as required
Qualifications:
- Education: BA/BS, Human Services field preferred
- Experience working with low-income/immigrant populations
- bilingual English/Spanish preferred
- Knowledge of community resources
- Excellent oral and written skills
- Ability to work a flexible schedule
- Good time management skills, able to handle multiple tasks and assignments at the same time
- Ability to work with a diverse population
- Self-starter
- Proficient with Microsoft Office programs, data/case management systems
