!!Calling all RN's, LPN's, & LSWs!!

Are you looking to have a Monday thru Friday, standard work schedule; NO WEEKENDS!

****CONTRACT TO HIRE****

Do you reside in or near Belleville or Marion Illinois? You must live IL for this role.

Do you have an Active and Unrestricted license in the State of Illinois?

Do you have a BA Degree in Social Work or Psychology with experinece in case management?

Do you have the discipline to work remotely, conduct home visits, uphold excellent Customer Service and Federal Guidelines?

JOB PURPOSE:

This position is responsible for conducting non-clinical assessments via phone (remote) and face to face (once COVID restrictions are lifted). Those experienced in case management and home health seem to be great fit.

PREFERRED JOB QUALIFICATIONS:

* At least three (3) years of Clinical experience under your license.

* Wellness or managed care (MCO) experience a plus!

* Home Health RN/LPN

* Condition Management experience

* Bilingual in English and Spanish (A plus but not required)

* Certification in Case Management, Training, Project Management or nationally recognized health care certification (A Plus but not required.)
