Job Title: Diabetes Specialist Nurse – Community Diabetes Service

Tripod Partners are currently recruiting for a Band 6 Diabetes Specialist Nurse to join the Community Diabetes Service in Peterborough. This is a full-time role, Monday to Friday, with the responsibility of managing a caseload of patients with complex diabetes needs.

Key Responsibilities

• Work as part of a multidisciplinary team to provide ongoing care for patients with diabetes, aiming to prevent hospital admissions and manage care in the community.
• Manage a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centred goals to assess, plan, implement, and evaluate interventions.
• Promote a better understanding of diabetes for patients, their families, carers, and other agencies involved through education, support, and counselling.
• Collaborate with the multidisciplinary team to ensure comprehensive care and support for patients.

Position Details:

• Location: Healthy Living Centre, Peterborough
• Work Pattern: Monday to Friday, 37.5 hours per week
• Contract Duration: ASAP start
• Rate: £26.50 per hour
• Pay Type: Umbrella or PAYE available

Requirements:

• Experience in diabetes care and management
• Enhanced DBS Check (adults) required
• Ability to work effectively within a multidisciplinary team and manage complex cases
