Diabetes Specialist Nurse

We are seeking a dedicated and knowledgeable Diabetes Specialist Nurse to promote the health and well-being of individuals with complex diabetes. You will empower patients to achieve optimal glycaemia control through informed self-management and provide essential support and education to both patients and caregivers. This role involves collaboration with healthcare professionals along the diabetes care pathway, acting as a vital link between GP and consultant-led specialist teams.

Key Responsibilities:

• Empower individuals with diabetes to manage their condition effectively, providing expert advice and support in both clinical settings and community outreach.
• Deliver comprehensive education programs for patients and their families, aiming to prevent complications and improve overall health outcomes.
• Collaborate with multidisciplinary teams to develop and implement effective care programs and ensure seamless communication across all levels of diabetes care.
• Maintain accurate patient records and participate in continuous professional development to enhance practice and patient care quality.

What We’re Looking For:

• 6 months of Healthcare experience as a Diabetes Specialist Nurse
• Right to work in the UK
• References covering the last 3 years of clinical employment
• NMC Registration
• Overseas Police Check, if any
• Indemnity Insurance
• Up-to-date CV
