This role might be for you if you’re an experienced palliative nurse who’s looking to: Work to your full RN scope of practice Work on a small, friendly & fun team Teach, mentor & coach others everyday Work across all sectors of healthcare Take initiative & practice independently Be creative & design HPC education Practice work-life balance POSITION OVERVIEW HPC Consultation Services provides case based consultation, education and mentoring for healthcare providers (HCP); assists HCPs in the application of the Model to Guide Hospice Palliative Care, assessment tools and best practice guidelines; helps build capacity within front line HCPs in the delivery of palliative care; offers consultation to HCPs in person (as allowed by Public Health status), by telephone, through e-mail and/or video conferencing regarding care, e.g., assessment and management of pain and other symptoms ; and links providers with specialized hospice palliative care resources. The Palliative Nurse Consultant applies expertise in palliative pain and symptom management in providing case consultation, education, mentorship and linkages to evidence informed resources. The Nurse Consultant will collaborate, develop and maintain effective working relationships within the long-term care (LTC) homes, hospices, local Home & Community Care Support Services (H&CCSS) and contracted agencies, acute care facilities, primary care providers and community support agencies. REQUIRED QUALIFICATIONS AND EXPERIENCE Bachelor’s Degree in Nursing, registered in good standing with the College of Nurses of Ontario Canadian Nurses’ Association Certification in Hospice Palliative Care (CHPCN(C)) Minimum of 3 – 5 years’ experience practicing in palliative care Successful completion of Comprehensive Advanced Palliative Care Education (CAPCE) preferred Possession and maintenance of a valid ON Driver’s License and vehicle insurance with access to a reliable vehicle and an ability to drive within the Waterloo Wellington area SKILLS AND ATTRIBUTES Advanced assessment and clinical reasoning skills Excellent communication skills including providing detailed explanations and interpretation with courtesy, tact, and diplomacy. Ability to adapt to changing requirements and environments whether consulting or providing education (virtual & in-person) Detailed understanding of the healthcare system including H&CCSS, acute care hospitals, LTC, primary care and community support services Ability to plan, prioritize, coordinate and implement complex cross-functional projects Ability to manage time constraints, conflicting demands and quality pressures Committed to continuous professional development Self-motivated, organized, and compassionate Ability to teach healthcare providers utilizing in person, virtual and blended teaching methods User knowledge of the D2L Brightspace Learning Management System an asset POSITION SPECIFIC RESPONSIBILITIES Clinical Consultation and Education o Act in a clinical consultant role concerning palliative care and symptom management to healthcare providers at all levels across the continuum of care o Serve as a mentor to inspire, develop, motivate and coach front line staff and leaders to deliver a palliative approach to care o Contribute to building program capacity including strategies for developing and maintaining key relationships, developing regional resources and enabling access to program resources o Responsible for presenting a full range of educational programs which may include: the Fundamentals of Hospice Palliative Care (HPC), Advance Palliative Practice Skills (APPS), Comprehensive Advanced Palliative Care Education (CAPCE), Compassion Fatigue, Advance Care Planning, Essential Pain Management (EPM), Learning Essential Approaches to Palliative Care (LEAP), and other workshops using principles of adult education o Participate in and/ or facilitate community nursing provider palliative rounds as needed o Contribute content for use by the program o Promote and use best practice clinical guidelines in teaching and training o Develop and collect program and organizational metrics to support reporting to internal and external partners  External Relationship Management o Develop and cultivate rapport to build and foster trust and credibility with partners and the wider community o Support the Program Manager with the planning and execution of strategic initiatives o Participate at local, regional and provincial levels on relevant committees or projects at the discretion of the Program Manager
