Job Description

Locum Band 7 Community Diabetes Nurse

* £35 per hour

* 22.5 hours per week between Mon - Fri 9-5 or 8-4 (7.5 hours per day)

* 6-month contract

* Must be able to drive and have own transport

* Camberley, Surrey

We are seeking an experienced and knowledgeable Locum Band 7 Community Diabetes Nurse to join the community healthcare team based in Camberley, Surrey. The successful candidate will specialize in providing expert diabetes care and support to patients in community settings. The role involves delivering comprehensive diabetes management, education, and self-care strategies to patients, while also working with a multidisciplinary team to ensure high-quality care and improved patient outcomes.

Patient Assessment and Management

* Provide expert assessment and management of patients with Type 1, Type 2, and gestational diabetes in the community.

* Develop and implement individualized care plans to optimize blood glucose control and prevent complications.

* Adjust and manage insulin therapy, oral hypoglycemic agents, and other treatments as appropriate.

* Perform clinical tasks such as blood glucose monitoring, foot checks, and cardiovascular risk assessments.

Patient Education and Self-Management

* Deliver comprehensive diabetes education to patients and their families, covering diet, lifestyle changes, insulin administration, and medication adherence.

* Empower patients to manage their diabetes through self-care strategies, promoting long-term health and independence.

* Provide training on the use of insulin pumps, continuous glucose monitors (CGMs), and other diabetes-related technology.

Multidisciplinary Collaboration

* Work collaboratively with GPs, dietitians, endocrinologists, podiatrists, and other healthcare professionals to ensure coordinated diabetes care.

* Participate in multidisciplinary team meetings and case conferences to discuss patient care and progress.

* Facilitate referrals to specialist services, such as diabetic foot clinics, when necessary.

Complex Case Management

* Manage patients with complex diabetes care needs, including those with co-morbidities, mental health challenges, and complications such as diabetic retinopathy or nephropathy.

* Provide support and guidance for patients transitioning between hospital and home care settings.

* Offer advice and support in the management of hypoglycemia, hyperglycemia, and diabetic ketoacidosis (DKA).

Documentation and Reporting

* Maintain accurate and up-to-date patient records, including assessments, care plans, interventions, and progress notes.

* Ensure all documentation complies with legal, ethical, and organizational standards.

* Provide detailed reports on patient outcomes and contribute to audit and quality improvement initiatives.

Professional Leadership and Supervision

* Provide clinical leadership and guidance to junior nurses, healthcare assistants, and students.

* Engage in continuing professional development (CPD) and reflective practice to maintain a high standard of care.

* Participate in diabetes-related training and education programs for healthcare staff in the community.

Health Promotion and Disease Prevention

* Lead diabetes prevention initiatives in the community, promoting healthy lifestyles and early intervention for at-risk populations.

* Organize and deliver educational sessions and workshops on diabetes management for patients and healthcare professionals.

Qualifications and Experience

Essential

Registered Nurse with current registration with the Nursing and Midwifery Council (NMC).

Specialist qualification or certification in diabetes care.

A minimum of 3-5 years of post-qualification experience, including experience in diabetes management.

Excellent clinical knowledge of diabetes care, treatment modalities, and patient education.

Strong communication, leadership, and interpersonal skills.

Ability to work independently and manage a caseload effectively.

Desirable

Experience working in a community or outpatient setting.

Previous experience in insulin pump therapy, CGMs, or diabetes technology.

Experience in leading diabetes health promotion initiatives
