What you'll be doing

The Aftercare Lead (the Lead) provides District-wide leadership for the integration of Aftercare services in accordance with Towards Zero Suicides initiatives. The Lead provides community-based assertive psychosocial support for people following a suicide attempt or suicidal crisis who have been referred from an acute care or community mental health setting.

This role will lead the coordination of Aftercare services with relevant hospital and community mental health and suicide prevention services to ensure access to aftercare support is available for all people who need it.

The Lead will be required to plan and implement strategies to enhance service visibility for consumers, families, carers, and clinicians, coordinate with District teams to ensure the effectiveness of referral pathways, and play a lead role in clinical and operational governance consistent with NSW Health and ISLHD policies, procedures and standards.

This role is a Category A role. Preferred applicants will be reviewed for compliance and required to maintain compliance with the screening, assessment and vaccination requirements, relevant to your position, as outlined in NSW Health policy directive on Occupational Assessment, Screening and Vaccination against Specified Infectious Diseases.

For positions designated as child-related employment, or positions that may result in proximity to children, you will be required to obtain a Working with Children clearance number from the Office of the Children’s Guardian.

To obtain a WWCC clearance number, follow the instructions on the website of Service NSW, which includes completing an online application form and then attending a Service NSW Centre or a NSW Council Agency that provides Road and Maritime Services, to provide proof of identity and pay for the WWCC to be completed.

Please note that a volunteer WWCC (which is free) cannot be used for paid employment.

Selection Criteria

  1. Current registration with AHPRA as a Registered Nurse with a minimum of 5 years post registration experience and an appropriate clinical post graduate qualification in the specified field and/or experience. Psychologist or Allied Health Professional must have current registration with the Australian Health Practitioner Regulation Agency (if applicable) and/or eligibility for membership of a relevant peak body professional association.
  2. Proven experience in complex and multi-site project and change management in a health setting, preferably in mental health and suicide prevention.
  3. Demonstrated experience in mental health and suicide prevention and safety planning with at least 3 years mental health experience.
  4. Demonstrated ability to work collaboratively with senior health managers, clinical leaders, research partners and other external stakeholders. This includes facilitation and negotiation skills such as the ability to resolve conflicts, manage competing demands and prioritising resources.
  5. Experience in use of routinely collected clinical information for service improvement, research and/or performance management, including issues related to data governance.
  6. Highly developed analytical skills and demonstrated ability to interpret, analyse and communicate complex information rapidly and accurately using verbal and written communication.
  7. Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
  8. Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities.
