Clinical Triage and Rehab Support Practitioner

Location: Leatherhead

Job Type: Permanent – Part-Time – 4 days a week

Sector: Healthcare

Base: Hospital

Salary: £35,392 to £45,996 a year plus excellent benefits


Duties

To be professionally and legally accountable for all aspects for your own and delegated work.

To work within codes of practice and professional guidelines.

To undertake specialist holistic rehab assessment (including those with complex presentations and multi pathologies) making use of clinical reasoning skills. This may include face assessment or telephone assessment / triage.

To control and manage triage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.

To evaluate patient progress and / or change, reassess and understand the clinical rationale for altering a rehabilitation plan and interventions

To understand and be acutely aware of significant medical observations during assessment and treatment of patients and know who to report this to

To complete a written and thorough assessment of all clients requiring an in person assessment

To work with the business development team, and admissions and discharge coordinator, sharing findings of your assessment in a proactive and timely manner so to support effective and efficient communication to referrers regarding outcome of clinical triage and the safe admission of clients.

To maintain an effective and efficient communication line with HOCS to discuss any complex clients or assessments, ensuring that decisions are not made in silo and the appropriate clinical guidance is obtained.

To maintain up-to-date clinical skills and rotate into clinical team as appropriate to support client rehab and / or care and maintain clinical skills. Clinical skills will be utilised for complex and needed face assessment of referrals and clinic support

To be responsible for the handover process to the relevant QEF CRC clinical staff regarding outcome of clinical triage and /or face assessment, including recommendations for therapy input, and appropriate rehab program with the aim to create a smooth and effective transition of care from clients place of residence into to the CRC

To demonstrate the importance of gaining patient and where appropriate family, carer or additional professional consent for all interactions and assessments.

To effectively communicate with patients, family members, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication, e.g. hearing loss, diminished sight, depression, aphasia, speech problems, cognitive impairment, behavioural problems and pain

To work in line with QEF policies and procedures.

To work within your own professional body such as the RCN, CSP or RCOT, demonstrating a good working knowledge of national and local standards of practice in keeping with legal and operational requirements

Organise and manage own time in keeping with line manager expectations.

To conduct and contribute to risk assessments and any relevant health and safety assessments that may be required for new client admissions

To be responsible for the handover process to the relevant QEF CRC staff of resulting recommendations from risk and health and safety assessments.

Take an active part in the development of the QEF CRC service and in supporting the organisational aims and objectives regarding models of care and clinical pathways.

To work effectively within the service and liaise appropriately with members of the MDT.

To ensure that accurate and timely written records are kept which comply with the QEF policy and HCPC guidance, reporting on any issues as appropriate.

To participate in the on-going development of the role and support the development of the protocols and processes in the Triage

To support the HOCS, General Manager and therapy team leads with day-to-day planning and delivery of a high quality service.

Participate in annual audit plans for the Service and annual staff surveys.

To undertake evidence based practice to further own clinical practice and maintain competency.

To comply with the collection of data required by the service including SSNAP data.

To participate in clinical supervision

To participate in the staff appraisal scheme and personal development plans



Qualification, Skills and Experience

Essential

5 years + post qualification clinical experience

3 years + Community experience

3 years + Experience with neurologically impaired adults

Solid experience of working in a team / MDT environment

Experience of working in a rehab setting

Experience of working autonomously

Experience of carrying a caseload

Experience of making decisions based on clinical rationale

Desirable

Experience of working in a customer focused environment


Essential

Registered nurse or post graduate physiotherapist / occupational therapist

Current NMC or HCPC registration

Completed Mentorship and / or clinical supervision training

Evidence of ongoing professional development

Desirable

Post graduate training Msc, MSc modules, PGDip, PGcert
