As part of our dynamic team, you can lead the way, as we continue our journey towards high reliability care We currently have an exciting opportunity for a Nurse Practitioner to join our Critical Care, Cardiology, Oncology and Respiratory Therapy team. The Nurse Practitioner (NP) in collaboration and partnership with the physicians and other members of the interprofessional team will provide specialized and advanced nursing care for both the outpatient Heart Function Clinics and the inpatient medicine units as appropriate. The NP provides leadership, and is responsible for the promotion and enhancement of cardiac care across the health care continuum. Responsibilities: Integrates research into practice, to ensure best practice standards are maintained and incorporated into clinical initiatives. Contributes to the education and professional development of nursing and the interprofessional team, and to the wider professional community within their area of specialization. Initiates and leads best practice initiatives that promote improved patient outcomes in their specialty area. Creates, utilizes and/or disseminates new nursing knowledge and participates in the design of the Best care for cardiac patients. Provides services that reflect the Mission, Vision and Values of Humber River Health and the surrounding diverse community. Provides direct, specialized cardiac care and/or consultation to patients, families, health care professionals, and community partners in situations where advanced nursing expertise is required (e.g., complex patient situations with unpredictable outcomes). Collaborates with the Medicine and Cardiology team in identifying goals of care and advance end of life care planning. Provides leadership in establishing and meeting goals related to nursing and clinical practice; accountable to align goals with the strategic direction of the program and the organization. Initiates and performs diagnostic and therapeutic procedures for identification and management of health problems as authorized through medical directives. Educates health care team members in best practice standards in the management of patients who demonstrate symptoms of heart failure, acute coronary syndromes, etc. Develops, implements, and evaluates effectiveness of clinical service programs in collaboration with the Program Director and Director of Professional Practice & Chief Nursing Executive. Engages in or conducts nursing research and participates in interprofessional research projects, as required. Engages in scholarly activities, such as presentations and publications. The Nurse Practitioner works in a collaborative practice model with physicians, nurses and inter-professional team members to provide quality care to patients and their families within the area of cardiac nursing to achieve quality clinical outcomes through evidence based practice. Initiates and manages the care of paitents and/or monitors the ongoing therapy of patients/clients with chronic stable illness. Monitors client health maintenance in partnership with the patient/client and family. Collaborates with the client and family and the interprofessional team to develop health care. Requirements: Masters Degree in Nursing and completion of an Acute Care Nurse Practitioner Program required. CNA certification in Cardiology preferred. Current registration in the Extended Class (NP - Adult) in good standing with the College of Nurses of Ontario required. Member of the Registered Nurses Association of Ontario preferred, member of NPAO required. Current Advanced Cardiac Life Support certification required. Minimum 3years experience in adults, outpatient and/or community nursing services, preferred. Demonstrated Clinical experience and proficiency in cardiac patient management and clinical consultation for the complex frail elderly. Advanced assessment, diagnostic reasoning skills. Knowledge of the principles of adult education and excellent verbal and written presentation skills Strong interpersonal communication skills and ability to establish positive relationships with all physicians, staff, patients/clients, families, and community agencies. Excellent creative self-directive and problem solving ability and critical thinking skills and able to work independently in collaboration with interprofessional team. Experience with research methodology, program planning and evaluation. Experience in Change Management an asset. Proficiency with computer applications, including electronic medical records. Strong knowledge of Quality improvement processes and Lean process improvement Excellent attendance and discipline free record required.
