We have an excellent opportunity available for a Mental Health Social Worker to join our Integrated Mental Health Service at Wokingham Borough Council, on a full time, permanent basis.



Following a service review, Wokingham Borough Council (WBC) and Berkshire Healthcare NHS Foundation Trust (BHFT) have restructured the delivery of mental health services within the integrated mental health service for adults.



Social care will introduce a dedicated social care pathway, while BHFT will provide health care under the One Team and care pathways model of care delivery. Although WBC staff will manage social care and BHFT will provide health care separately, the service will collaborate closely as part of a multidisciplinary team to ensure the best outcomes for service users.



WBC will continue to support individuals with complex mental health needs; however, this is need based rather than via the diagnostic secondary care gateway.



The model of delivery will involve greater collaboration with WBC locality and commissioning services, as well as increased engagement with VCSO and Third sector organisations.

Key responsibilities:



Working as part of a multi-disciplinary team the successful candidate will be responsible for working with individuals with severe mental illness for a social care perspective. This will include giving advice and signposting to VCS organisations, promotion of wellbeing, carrying out Care Act assessments and S117 MHA assessments, compiling support plans and working with the operational commissioning team to commission care to meet eligible needs.



Undertaking adult safeguarding enquiries and carrying out mental capacity assessments.



Candidate requirements:




  • Essential: qualified social worker, registered with SW England and has completed ASYE with sound knowledge of the Care Act 2014

  • Essential: good understanding of adult Safeguarding, Mental Health Act, Mental Capacity Act, Care Act, and Human Rights legislation

  • Essential: Full driving license and own transport

  • Desirable: qualified as a BIA or AMHP

  • Desirable: Experience in working with adults with severe mental illness
